Support that Empowers

For Brighter Futures
At CJP Outreach Services, we understand that every person has unique strengths, aspirations and support needs. Our experienced teams work alongside individuals, families and professionals to deliver personalised support that enhances quality of life, promotes independence and achieves meaningful outcomes.
We provide specialist support for people with complex and diverse needs, ensuring support is tailored to the individual, not simply the diagnosis. Through compassionate, outcome-focused support, we empower people to lead fulfilling lives whilst remaining at the centre of every decision.
At CJP Outreach Services, we understand that behaviours of concern are often a form of communication, reflecting unmet needs, anxiety, frustration, pain or difficulties in expressing thoughts and feelings.
Our approach focuses on understanding the person, their experiences and the factors influencing behaviour, rather than simply responding to incidents as they occur. By building positive relationships, creating consistent support strategies and promoting emotional wellbeing, we help individuals feel safe, understood and empowered.
Our highly trained teams utilise evidence-based approaches, including Positive Behaviour Support (PBS), Studio III and other specialist frameworks, to develop proactive strategies that reduce distress, improve quality of life and increase opportunities for independence and community participation.
Working collaboratively with individuals, families and professionals, we focus on creating sustainable, positive outcomes that help people achieve their goals and live fulfilling lives.
At CJP Outreach Services, we believe that every behaviour serves a purpose and communicates a need. Our Positive Behaviour Support (PBS) Plans are designed to help individuals, families and support teams better understand the factors that influence behaviour, enabling proactive and person-centred support.
Our PBS Plans focus on understanding the individual as a whole, recognising their strengths, preferences, communication style, environment and life experiences. By identifying triggers, patterns and unmet needs, we can develop practical strategies that reduce distress, improve wellbeing and enhance quality of life.
Working collaboratively with the individual, their support network and relevant professionals, we create bespoke PBS Plans that promote consistency, build resilience and support meaningful outcomes.
Our PBS Plans provide a structured, evidence-based approach to understanding behaviour and promoting positive outcomes. Developed in partnership with individuals, families and professionals, each plan identifies proactive strategies that reduce distress, enhance quality of life and support greater independence.
We believe that exceptional support starts with exceptional training. Our teams receive specialist training that equips them with the skills, knowledge and confidence to provide safe, compassionate and effective support for people with complex needs.
We deliver evidence-based training programmes including Positive Behaviour Support (PBS), NAPPI and Studio III, helping colleagues develop positive, proactive approaches that promote wellbeing, reduce distress and improve quality of life.
Training is never viewed as a one-off event. We are committed to ongoing learning and development, ensuring our teams remain responsive to changing needs, best practice guidance and the unique circumstances of every individual we support.
Every person is different, which is why we invest in training that reflects the needs of the people we support. Alongside PBS, NAPPI and Studio III, we can provide a range of additional specialist training programmes designed around specific health, behavioural, communication and support requirements.
By combining specialist training with person-centred values, we ensure our teams are equipped to provide consistent, high-quality support that promotes independence, dignity and meaningful outcomes.
We use recognised evidence-based frameworks and quality-of-life measures to understand what matters most to the people we support.
By incorporating tools such as the PERMA model and outcome-focused assessments, we can develop personalised support strategies that promote wellbeing, independence and meaningful outcomes. Ultimately, our goal is simple: to ensure that support is shaped by what matters most to the individual, resulting in better experiences, better outcomes and a better quality of life.
Using Positive Behaviour Support, specialist training and person-centred approaches, we focus on understanding the factors that contribute to distress and developing proactive strategies that reduce the need for restrictive interventions whilst promoting safety, independence and quality of life. We believe that reducing restrictive practices is about more than managing risk. It is about increasing opportunities, promoting independence, strengthening relationships and enabling people to live the life they choose.
We believe the best outcomes are achieved when individuals, families, professionals and support teams work together. Through open communication, shared decision-making and regular review, we ensure everyone has a voice and remains informed. By building trusted partnerships and maintaining transparency at every stage, we create support that is consistent, responsive and centred around the individual’s goals and aspirations. Strong partnerships are at the heart of effective support. We work collaboratively with individuals, families and professionals.
We believe that consistency is essential. By building stable, dedicated support teams, we help people develop trusting relationships with those who know them best.

Our support teams receive specialist training tailored to the needs of the people we support, including complex health needs, autism, behaviours of distress, communication, sensory needs and positive behaviour support.

transforming Care Complex Nurse - Somerset
I am pleased to provide this statement in support of CJP, who have consistently demonstrated exemplary standards in the care and support of the individuals with complex needs and behaviours that challenge.
CJP have provided an outstanding service to two of my patients under the care of the Rapid Intervention Team within Somerset NHS Foundation Trust. Both individuals present with highly complex behavioural profiles that previously led to placement breakdowns with other providers. In contrast CJP not only sustained these packages of care, but they made demonstrable progress in promoting stability, engagement and improved quality of life.
Their approach is person centred, highly progressive and underpinned by a genuine commitment to joint working. CJP are a model of collaborative, multi disciplinary practice, always approachable, transparent, and proactive in communication. They work closely with NHS and Social Care professionals, and are consistently receptive to advice, feedback and co produced care planning

experienced Social Worker
As I mentioned in our meeting, I feel that CJP Community Services – and you in particular – have been exceptional in coordinating the response to GM’s complex health issues.
As an out-of-county Social Worker – being so far away – it is reassuring to be kept in the communication loop and to know that you are liaising with all relevant professionals, health, social care and Advocacy. Your efforts in coordinating everyone involved will ensure the best outcome for someone who potentially has a serious health condition but lacks the mental capacity to make informed decisions about this.
This proactive and collaborative approach not only safeguards the individual but also demonstrates strong compliance with CQC standards. This reflects best practice in ensuring health needs are addressed promptly and in a person-centred manner. Such an approach is essential for GM’s health and wellbeing, while also supporting quality assurance and meeting regulatory requirements
