For Brighter Futures
At CJP Outreach Services, we understand that every person has unique strengths, aspirations and support needs. Our experienced teams work alongside individuals, families and professionals to deliver personalised support that enhances quality of life, promotes independence and achieves meaningful outcomes.
We provide specialist support for people with complex and diverse needs, ensuring support is tailored to the individual, not simply the diagnosis. Through compassionate, outcome-focused support, we empower people to lead fulfilling lives whilst remaining at the centre of every decision.
Mental health affects people in different ways and can influence how they think, feel and interact with the world around them. At CJP Outreach Services, we recognise that every person’s experiences are unique and that support should reflect their individual circumstances, strengths and aspirations.
We take the time to understand the person behind the diagnosis, building positive relationships based on trust, respect and genuine understanding. By listening to what matters most to each individual, we can provide support that is meaningful, personalised and focused on achieving positive outcomes.
Our approach promotes stability, confidence and emotional wellbeing. We encourage individuals to develop coping strategies, build resilience and engage in activities that support their recovery and overall quality of life.
Whether someone requires short-term assistance or ongoing support, our goal remains the same: to empower people to take positive steps forward, achieve greater independence and live fulfilling lives on their own terms.
We understand that no two mental health journeys are the same. That’s why every support plan is developed around the individual’s specific needs, goals, preferences and aspirations.
Working alongside the individual, their family, advocates and healthcare professionals, we create personalised plans that provide clear guidance and promote consistency in the support being delivered. This collaborative approach ensures that support remains focused on what is important to the person.
Our plans identify strengths as well as areas where additional support may be beneficial. By recognising both opportunities and challenges, we can develop practical strategies that promote confidence, recovery and increased independence.
Support plans are regularly reviewed to ensure they continue to meet the individual’s changing needs and aspirations. This enables us to provide responsive support that evolves alongside the person and helps them achieve meaningful and lasting outcomes.
Delivering effective mental health support requires knowledge, understanding and compassion. Our teams receive specialist training that equips them with the skills and confidence needed to support individuals experiencing a wide range of mental health challenges.
Training focuses on developing a deeper understanding of mental health conditions, recovery-focused approaches, trauma-informed practice and person-centred support. This helps staff build positive relationships and provide support that reflects each individual’s unique circumstances.
Our teams are trained to recognise early signs of distress, respond appropriately to changing needs and promote strategies that support emotional wellbeing. By taking proactive and supportive approaches, we help create environments where people feel safe, respected and understood.
Through ongoing learning and professional development, we maintain a highly skilled workforce committed to delivering high-quality support. This combination of expertise and compassion enables us to help individuals achieve positive outcomes, greater independence and improved wellbeing.
Recovery looks different for everyone. We work alongside individuals to help them identify personal goals, develop coping strategies and build the confidence needed to move forward positively. Through consistent, person-centred support, we help people recognise their strengths, increase resilience and take greater control of their wellbeing, promoting long-term stability and independence.
Positive mental health is about more than managing challenges. Our support focuses on helping individuals improve their overall wellbeing, build self-confidence and participate more fully in everyday life. By encouraging choice, developing life skills and creating opportunities for personal growth, we empower people to live fulfilling lives and achieve outcomes that are meaningful to them.
Effective support is built on trust, communication and collaboration. We work closely with individuals, families, healthcare professionals and wider support networks to ensure support remains joined-up, responsive and focused on achieving positive outcomes. transforming Care Complex Nurse - Somerset
I am pleased to provide this statement in support of CJP, who have consistently demonstrated exemplary standards in the care and support of the individuals with complex needs and behaviours that challenge.
CJP have provided an outstanding service to two of my patients under the care of the Rapid Intervention Team within Somerset NHS Foundation Trust. Both individuals present with highly complex behavioural profiles that previously led to placement breakdowns with other providers. In contrast CJP not only sustained these packages of care, but they made demonstrable progress in promoting stability, engagement and improved quality of life.
Their approach is person centred, highly progressive and underpinned by a genuine commitment to joint working. CJP are a model of collaborative, multi disciplinary practice, always approachable, transparent, and proactive in communication. They work closely with NHS and Social Care professionals, and are consistently receptive to advice, feedback and co produced care planning

experienced Social Worker
As I mentioned in our meeting, I feel that CJP Community Services – and you in particular – have been exceptional in coordinating the response to GM’s complex health issues.
As an out-of-county Social Worker – being so far away – it is reassuring to be kept in the communication loop and to know that you are liaising with all relevant professionals, health, social care and Advocacy. Your efforts in coordinating everyone involved will ensure the best outcome for someone who potentially has a serious health condition but lacks the mental capacity to make informed decisions about this.
This proactive and collaborative approach not only safeguards the individual but also demonstrates strong compliance with CQC standards. This reflects best practice in ensuring health needs are addressed promptly and in a person-centred manner. Such an approach is essential for GM’s health and wellbeing, while also supporting quality assurance and meeting regulatory requirements
