Celebrating 20 Years of Supporting People

Helping People Communicate, Connect and Thrive

At CJP Outreach Services, we understand that every person has unique strengths, aspirations and support needs. Our experienced teams work alongside individuals, families and professionals to deliver personalised support that enhances quality of life, promotes independence and achieves meaningful outcomes.

We provide specialist support for people with complex and diverse needs, ensuring support is tailored to the individual, not simply the diagnosis. Through compassionate, outcome-focused support, we empower people to lead fulfilling lives whilst remaining at the centre of every decision.

Communication is different for everyone. Some people communicate using words, while others may use signs, technology, symbols, behaviours or non-verbal methods to express themselves. We take the time to understand each person’s preferred way of communicating and ensure their voice is heard.

Building meaningful relationships starts with effective communication. By adapting our approach to the individual’s needs, we help create positive interactions, strengthen connections and promote greater confidence in everyday life.

We work closely with individuals, families and professionals to develop communication approaches that are effective, consistent and person-centred. This helps reduce frustration and creates more opportunities for meaningful engagement.

By recognising and respecting individual communication preferences, we empower people to express themselves, make choices and participate more fully in the activities and experiences that matter to them.

Understanding Communication, Supporting Connections

Personalised Sensory Support Plans

Everyone experiences sensory information differently. Some individuals may be more sensitive to noise, light, touch, taste or movement, while others may seek additional sensory input to feel comfortable and regulated.

Our personalised support plans consider each person’s unique sensory preferences, triggers and coping strategies. By understanding these needs, we can create supportive environments that promote wellbeing and reduce unnecessary stress or anxiety.

We work collaboratively with individuals and those around them to identify what works best, ensuring support remains consistent and responsive to changing needs.

Through careful planning and person-centred approaches, we help people feel more comfortable, confident and able to engage positively with the world around them.

Our teams receive specialist training designed to develop a deeper understanding of communication differences and sensory processing needs. This enables staff to provide effective, responsive and person-centred support.

Training focuses on recognising individual communication styles, understanding sensory experiences and adapting support to meet the needs of each person. This helps create positive environments where individuals feel understood and respected.

By combining knowledge with compassion and practical experience, our teams can identify potential barriers and implement strategies that promote positive outcomes and greater independence.

Through continuous learning and development, we maintain a skilled workforce that is committed to delivering high-quality support that enhances wellbeing, confidence and participation.

Working Together in Partnership

Effective support is built on collaboration. We work closely with individuals, families, therapists and other professionals to develop shared approaches that promote consistency and positive outcomes. By maintaining open communication and strong partnerships, we ensure support remains responsive, person-centred and focused on helping individuals reach their full potential.

How We Build Outstanding Support

1

Specialist Training That Builds Confidence

Delivering exceptional care and support starts with exceptional training. Our colleagues receive specialist training tailored to the unique needs of the people they support, ensuring they have the knowledge, skills and confidence to provide safe, effective and person-centred care.

Through continuous learning and development, we equip our teams to respond positively to complex situations and changing needs.

2

Investing in Expertise, Delivering Better Outcomes

We believe that high-quality support is driven by well-trained and knowledgeable teams. Our staff undertake specialist training in areas including complex care, autism, behaviours of distress, communication, sensory needs and positive behaviour support.

This investment enables us to deliver consistent, high-quality support that helps people achieve meaningful outcomes and greater independence.

3

Specialist Training, Consistent Teams

Strong relationships are built through consistency and expertise. We carefully develop dedicated support teams who receive specialist training tailored to the individuals they support.

This combination of training and continuity helps build trust, reduces anxiety and creates the stability needed for people to thrive.

transforming Care Complex Nurse - Somerset

Rated 5 out of 5

I am pleased to provide this statement in support of CJP, who have consistently demonstrated exemplary standards in the care and support of the individuals with complex needs and behaviours that challenge.

CJP have provided an outstanding service to two of my patients under the care of the Rapid Intervention Team within Somerset NHS Foundation Trust. Both individuals present with highly complex behavioural profiles that previously led to placement breakdowns with other providers. In contrast CJP not only sustained these packages of care, but they made demonstrable progress in promoting stability, engagement and improved quality of life.

Their approach is person centred, highly progressive and underpinned by a genuine commitment to joint working. CJP are a model of collaborative, multi disciplinary practice, always approachable, transparent, and proactive in communication. They work closely with NHS and Social Care professionals, and are consistently receptive to advice, feedback and co produced care planning

AB

experienced Social Worker

Rated 5 out of 5

As I mentioned in our meeting, I feel that CJP Community Services – and you in particular – have been exceptional in coordinating the response to GM’s complex health issues.

As an out-of-county Social Worker – being so far away – it is reassuring to be kept in the communication loop and to know that you are liaising with all relevant professionals, health, social care and Advocacy. Your efforts in coordinating everyone involved will ensure the best outcome for someone who potentially has a serious health condition but lacks the mental capacity to make informed decisions about this.

This proactive and collaborative approach not only safeguards the individual but also demonstrates strong compliance with CQC standards. This reflects best practice in ensuring health needs are addressed promptly and in a person-centred manner. Such an approach is essential for GM’s health and wellbeing, while also supporting quality assurance and meeting regulatory requirements
