Its core purpose is to ensure people receive the right support at the right time—especially when they are navigating multiple services (e.g., health, housing, social care, education, and employment).

Managing Complexity

We take a proactive, compassionate and competent approach to managing and recruiting complex support packages for our clients. Our aim is to provide practical and emotional support to empower our clients to take charge of their lives and progress towards a brighter future.


Assessment and Planning

We undertake a thorough evaluation of an individuals needs, strengths, risks and goals, and create a personalised support plan that is developed in collaboration with the individual, which sets clear, realistic goals and outlines the service and support required to achieve them


Coordination and Implementation

We support individuals to ensure that there is a link and organisation to additional services and professionals, ensure that everyone works together smoothly, avoiding duplication and gaps. We ensure that plans are put in to action, and that there are regular checks against the plan to monitor and review

Positive Support Plans

  • Our Support Plans detail an individual’s health care needs, including any diagnosis that they may have, and the treatment required. 
  • We write Support Plans in first person, and our support plan is centred around the individual and their wishes
